Types

- cANCA: diffuse cytoplasmic staining by immunofluorescence

  • serine proteinase-3 (PR3) is the predominant antigen for C-ANCA
  • Wegener granulomatosis (80-95%)

- pANCA: perinuclear staining

  • P-ANCAs are usually directed against myeloperoxidase (MPO)
  • Wegener granulomatosis (5-20%)
  • microscopic polyangiitis ++

- atypical pattern

  • inflammatory bowel disease
  • systemic immune-mediated diseases
  • infections
