 cords/nests basaloid cells
 sclerotic stroma
 horn cysts with keratin

Differential diagnosis

 cutaneous slerodermiform basal cell carcinoma
 microcystic adnexal carcinoma (MAC)
 syringoma

Synopsis

 Combination of microcysts and thin strands of basaloid epithelium wrapped by collagen
 Microcysts
 Foci of calcification (calcifeid keratin)
 Small strands of epithelium occasionally wrapped by small collagen fibers.
 The epithelial component resembles the mantle portion of the follicle.
 Other follicular constituents may be present.
 Possible clusters of ghost cells (as in pilomatricoma)
 Possible clusters of basaloid cells with fibroblastic stroma

Differential diagnosis

 sclerosing epithelial neoplasms

  • morpheiform basal cell carcinoma
  • syringoma
  • microcystic adnexal carcinoma
  • desmoplastic basal cell carcinoma
  • microcystic adnexal carcinoma

 ghost cell cutaneous tumors

  • pilomatricoma
