Definition : Exophytic lesions of the tongue encompass a diverse spectrum of entities. These are most commonly reactive, arising in response to local trauma but can also be neoplastic of epithelial, mesenchymal or miscellaneous origin. In most cases, the microscopic examination is likely to provide a straightforward diagnosis.

primary lingual colonic-type adenocarcinoma

Primary colonic-type adenocarcinoma involving the tongue (CTAT) is exquisitely rare.
The mass consists of invasive adenocarcinoma associated with “dirty necrosis”, akin to the phenotype seen in colorectal adenocarcinoma.
By immunohistochemistry, the tumor cells are positive for AE1/3+ , CDX2+ , CK20+ , SATB2+ and beta-catenin+ (CTNB1+ ).
A metastatic disease muste be excluded before rendering a diagnosis of CTAT.
