Atypical polypoid adenomyoma is biphasic: it has a stroma composed of fascicles of smooth muscle, and numerous atypical epithelial glands that have architecturally lost polarity but are not pleomorphic, as seen here. There are minimal mitotic figures, and squamous metaplasia is frequently found.

Atypical polypoid adenomyoma is composed of 1) fascicles of smooth muscle, 2) atypical epithelial glands, and (frequently) 3) squamous metaplasia.
