Synopsis

 Presents as a mass:

  • most common in mediastinum
  • also described in: neck, lung, axilla, mesentery, broad ligament, retroperitoneum, soft tissues of extremities, nasopharynx, meninges, several other sites

 morphological types

  • >90% are hyaline-vascular type Castleman disease: usually asymptomatic
  • ≈10% plasma cell type Castleman disease: often associated with: fever, anemia, elevated erythrosedimentation rate, hypergammaglobulinemia, hypoalbuminemia

 Sometimes microscopic changes suggesting early stage of same process in adjacent nodes

Macroscopy

 Round
 Well-circumscribed
 Cut surface: solid, gray
 can measure ≥15cm diameter
