inflammatory pseudotumor

Definition: Inflammatory pseudotumors (IPTs) embrace a heterogeneous spectrum of reactive, infective, and neoplastic entities, that are characterized by a clinical mass composed of a histologic proliferation of spindle cells in a background of inflammatory cells and collagen fibers.

Inflammatory pseudotumor (IPT) is a heterogeneous group of lesions occurring in various organs, which is histologically characterized by fibroblastic and myofibroblastic proliferation with inflammatory infiltrate.

Inflammatory myofibroblastic tumor (IMT) is a neoplastic counterpart of IPT, which shows aberrant expression of ALK and its gene translocation.

Etiology

- inflammatory myofibroblastic tumor (IMT)

  • ALK+ inflammatory myofibroblastic tumor (ALK+ IMT)
  • ALK- inflammatory myofibroblastic tumor (ALK- IMT)

- dysimmune inflammatory pseudotumors

  • hyper-IgG4 disease (IgG4-associated inflammatory pseudotumor)
  • IgG4-associated sclerosing disease
  • IgG4-associated inflammatory pseudotumor

- infective inflammatory pseudotumors

  • mycobacterial inflammatory pseudotumor
  • cryptococcal inflammatory pseudotumor (#17895752#)
  • HHV8-associated inflammatory pseudotumor
  • EBV-associated inflammatory pseudotumor
    • EBV-associated splenic inflammatory pseudotumor

Physiopathology

Inflammatory pseudotumors (IPT) are mass-forming lesions that occur in patients of various ages, in various anatomic regions and organs, including the lung, liver, gastrointestinal tract, abdominal soft tissue, lymph node, and spleen.

IPT is histologically characterized by a proliferation of fibroblasts and myofibroblasts admixed with lymphocytes, plasma cells, eosinophils, and histiocytes.

However, the cell population varies from one tumor to another or even from one microscopic field to another in the individual tumor.
