Differential diagnosis

 benign serous papillary cystadenoma

  • may have focal areas of atypia suggestive, but not diagnostic of borderline tumor

 serous carcinoma

  • low grade tumors may have similar appearance, but have destructive stromal invasion

 mucinous borderline tumors

  • epithelium is mucinous;
  • mixed tumors have mucinous, squamous or endometrioid differentiation, often associated with endometriosis

 ovarian clear cell carcinoma

  • may have papillary features, but are unilateral, associated with endometriosis;
  • have nonhierarchical branching, monomorphic cells with classic features elsewhere, focal cytologic atypia (Am J Surg Pathol 2008;32:269)
