Synopsis

 recurrent urinary tract infections
 dysuria
 terminal hematuria
 cystic lesion behind bladder and above the prostate
 midline drop-shaped cyst originating within the prostate gland and separating from the seminal vesicles

Histopathology

 features consistent with a müllerian duct remnant.
